Free Developmental Screening for Children 0-5

Welcome to the Developmental Screening! We are excited and honored to have you here. We understand that these are exceedingly difficult times, but we at Family Support Network strive to continue to provide services to our community in a healthy and safe environment for our families, partners and staff members. As the caregiver of the child you plan to take to the screening, we ask you to evaluate your own health before you participate. If you have had a fever, cough, sore throat or muscle aches and pains, please DO NOT ENTER THE SCREENING SPACE. We will be taking temperatures before you begin the process. If you have any indication of a fever or any symptoms, you will be asked to attend a future screening. Below is an outline of how the process will go on the day of the screening. Please read it carefully and adhere to the guidelines. If you do not feel you are able to abide by our guidelines, you will be asked to leave for the safety and security of everyone involved:

  1. Upon entering the parking lot, you will be greeted by one of our staff members who will give you your registration packet. We will ask you for your cell phone number and we will assign you a number and an approximate appointment time. Please park your vehicle and stay inside. Complete your paperwork while you wait. If there is a significant lapse between your arrival time and your appointment, you may leave and return at your designated time, but if you are not here at the time of your appointment, we will move to the next family in line. You will still be able to go through the screening, but you must wait for the other families to cycle through.
  2. When we are ready for you to come to the screening area, we will text you. If you do not have a cell phone, a runner will come to your vehicle and invite you to the staging area. One caregiver and child will be allowed in the staging area at a time. If you brought more than one child, YOU MUST BE ABLE TO KEEP THEM IN THE STAGING AREA WITH YOU. CHILDREN WILL NOT BE ALLOWED TO ROAM ABOUT IN THE STAGING AREA.
  3. Both caregiver and child must ALWAYS WEAR A FACE MASK during the screening. Children under the age of 2 SHOULD NOT WEAR A MASK. If you do not have one, we will give you disposable masks. Please wear them. The screening space has been designed to maintain the norms of social distancing – please remain within the designated spaces and markings.
  4. The screening is divided into stations. You will be escorted from station to station. Do not leave or enter a space without your escort. We are staggering the times so that everyone can move from space to space with little or no interaction from others. Once in the station space, you will have 10 minutes to complete the screening and move to the next space.
  5. Upon completion of the stations, you will be escorted to the appropriate check out station. Please be sure to take all your packet materials with you and be sure to have them reviewed. If referrals are needed you will be instructed as to following steps.
